Admissions Criteria
• Patient/family agree to comfort-oriented care rather than curative treatment. • Patient has a primary care physician responsible for their medical care who is supportive of hospice care and services. • Patient has an identifiable caregiver. If able to provide self-care upon admission, patient agrees to an alternative plan when unable to perform self-care. • Holy Family Hospice accepts patients based on eligibility regardless of race, color, religion, national origin, age, sex, disability or source of payment for care.
Admissions Application
A primary care physician must provide us with an order to consult and admit a patient to Holy Family Hospice. This can be obtained by the patient, the primary caregiver, or through the social services or nursing staff at a nursing or assisted living facility where a patient resides. If a patient is living at home and needs assistance, please contact the admissions coordinator at the number listed below. Subsequent steps leading to admissions includes:
• Nursing review of patient's medical history and appropriateness for hospice care. • Patient and family education regarding Holy Family Hospice services. • Admission to hospice and establishment of an individualized plan of care.For further assistance, contact Katherine Peters, Admissions Coordinator:
