Holy Family Hospice serves our patients with a team of experienced nurses, physicians, social workers, religious, nursing assistants and volunteers. Nursing support and access to other hospice services is available 24 hours a day, 7 days a week through the hospice on-call system. Under the hospice benefit, Holy Family Hospice provides prescription drugs related to the terminal diagnosis, durable medical equipment, supplies and the therapies necessary to ensure comfort and quality of life.
Holy Family Hospice can provide the following levels of care: • Routine hospice home care • Continuous care • Respite care (as temporary relief for caregivers) • General inpatient care (for pain and symptom control)
Interdisciplinary Team ApproachUnder the direction of a licensed Medical Director, our hospice team combines the experience of RNs, LPNs, Nursing Assistants, Licensed Social Workers, Religious, Bereavement Counselors, and Volunteers. Together, this unique group of individuals create a plan that provides support by combining conventional and alternative therapies to address spiritual, emotional and physical needs.PhysicianIn a collaborative effort, our Medical Director participates in team conferences and serves as a liaison between Holy Family Hospice and physician members of the medical community. The Medical Director may also educate and take part in public relations endeavors in the medical community and attend meetings and other activities that support the mission of Holy Family Hospice. Nurse ClinicianThe care of Holy Family Hospice patients is supervised by an RN case manager. Services provided by the clinician include frequent visits, assessment of patient needs, medication management, and education. The Hospice case manager works closely with the physician to meet our patients’ changing needs and to improve their quality of life.
Licensed Social WorkerEmotional support and counseling, active listening, assistance with medical decisions and financial matters, identification of community resources, and grief support are just a few of the services provided to patients and their families by our licensed social workers.Spiritual CarePatients and their families are surrounded by caring individuals who offer a sense of peace and comfort in the hospice setting. We strive to meet spiritual needs through pastoral support, communication with our patients’ religious community, assistance with memorials and funeral arrangements, and grief support.Nursing Assistants & Volunteers Nursing Assistants are trained to provide patients with personal care, while volunteers offer companionship, emotional support, help in the home, and assistance with errands. |
